The Microsoft Exchange Server 2010 Management Pack for System Center Operations Manager monitors the Windows Application log on computers running Exchange 2010 and generates this alert when the events specified in the following Details table are logged.

Rule Name

Outlook Web App failed to initialize. A referenced forms registry folder doesn't exist. Review MSExchangeOWA event 1 for the cause.


This Error event indicates that Microsoft Office Outlook Web App did not initialize because the forms registry folder referenced in the event description does not exist. The forms registry folder contains files that you must have to display an item or folder referenced in an Outlook Web App HTTP request. If the required forms registry folders or files do not exist, users cannot access their mailbox using Outlook Web App.

User Action

To resolve this error, replace the forms registry folder with the default folder on the Exchange Server 2010 DVD. Follow these steps:

  1. Locate the forms registry folder in the Setup\ServerRoles\ClientAccess\owa\<version>\forms folder on the Exchange 2010 DVD.

  2. Copy the contents of the forms folder to the C:\Program Files\Microsoft\Exchange Server\V14\ClientAccess\Owa\<version>forms folder on the computer that is running the Client Access server role, where C:\ is the directory to which you installed Exchange 2010.
