The Microsoft Exchange Server 2010 Management Pack for System Center Operations Manager monitors the Windows Application log on computers running Exchange 2010 and generates this alert when the events specified in the following Details table are logged.
To learn more about this alert, in Operations Manager, do one or more of the following:
- From the Operations Console, double-click this alert, and then
click the General tab. Review the description of the alert
that includes the variables specific to your environment.
- From the Operations Console, double-click this alert, and then
click the Alert Context tab. Review the logged events that
meet the criteria of this Operations Manager alert.

Explanation
This Error event indicates that Microsoft Office Outlook Web App did not initialize because the small icon configuration file referenced in the event description does not exist. The small icon configuration file contains references to the icons used by each message type in Outlook Web App. If this file does not exist, users cannot access their mailbox using Outlook Web App.
User Action
To resolve this error, follow these steps to replace the small icon configuration file with the default file that is located on the Exchange Server 2010 DVD:
- Locate the SmallIcons.xml file in the
Setup\ServerRoles\ClientAccess\owa\<version> folder on the
Exchange 2010 DVD.
- Copy the SmallIcons.xml file to the C:\Program
Files\Microsoft\Exchange
Server\V14\ClientAccess\Owa\<version> folder on the computer
that is running the Client Access server role, where C:\ is the
directory to which you installed Exchange 2010.
